Medication Permission Form
If your child requires medication to be administered during the school day, you will need to fill out the form below and return it to the office.
Permission to walk home
If you would like your child to leave school at the end of the day or after clubs unaccompanied by an adult, you will need to fill out the form below and return it to the office in order for your child to be issued with a gate pass. Please complete and return to the School Office in person, or via office@christchurchjuniorschool.co.uk
